If the dashboard shows the extension as active, but the bulk order form does not appear on the storefront, the issue is usually related to page context, visibility, or product selection.
Follow the steps below to identify the cause.
Step 1: Check the page type
Each extension works only on a specific page type.
What to check
Product Page extension โ must be on a product page
Bulk Order Page extension โ must be on a page
If you are viewing the wrong page, the form will not appear.
Step 2: Check visibility rules
The extension can be active, but the form may still be hidden.
What to check
Customer is logged in (if required)
Customer matches visibility rules
Correct bulk order form is shown
Step 3: Check product selection
If no products are available, the form may appear empty or not render.
What to check
Products are included in the form
Collections contain products
Products and variants are active in Shopify
Step 4: Check the active theme
The extension is active only for the active theme.
What to check
You are viewing the active theme
The extension was not added to a different theme
Step 5: Check the page template
Especially for bulk order pages:
The page must use the correct template
The extension must be added to that template
If the template is changed, the extension may no longer appear.
Step 6: Test in a private window
Admin sessions can hide real visibility behavior.

Open a private/incognito window
Log in as a customer
Reload the page
