Bulk order form is not visible

Overview
If the bulk order form is not visible on the storefront, this is usually caused by visibility rules, theme setup, or form status.
Follow the checks below in order.
Step 1: Check if the form is enabled
The bulk order form must be enabled to appear.
What to check
Go to Bulk order forms
Make sure the form status is On
Step 2: Check dashboard status indicators
Open the app dashboard and verify:
Product Page Active is active (for product pages)
Bulk Order Page Active is active (for bulk order pages)
If the status is inactive, the extension is not added to the active theme.
Step 3: Check visibility rules
The form is shown only to customers who match the visibility rules.
What to check
Visibility type (all customers, logged-in, tags, or specific customers)
Customer login status
Customer tags or assignment
Step 4: Check the page you are viewing
Make sure:
You are viewing the correct page (product page or bulk order page)
The extension is added to that page
The page uses the correct template
Step 5: Check the active theme
Extensions are theme-specific.
What to check
The theme you are viewing is the active theme
The extension is added to that theme
Step 6: Test in a private window
Testing in a logged-in admin session can give incorrect results.
What to do
Open a private/incognito window
Log in as a customer (if required)
Visit the page again
Step 7: Check for conflicting bulk forms
Only one bulk order form can be shown to a customer.
If multiple forms exist:
Another form may override visibility
The expected form may not be shown
